I used Avidemux to re-encode, using "MPEG-4 ASP (Xvid4)" and disabling B-frames by using the 'Configure' button -> Motion Tab -> Advanced Simple Profile -> Max B Frames = 0. No check marks under 'Quarter Pixel' or 'GMC' here either. Quantization matrix = H263 or Mpeg didn't seem to matter. This is Avidemux 2.4.4 I'm using, by the way.
It's been quite a while since I played with MPEG-2 repair so I may be hazy on old details, but in a world of IPB frames isn't a lack of B frames a size liability?...
